(LTCFs) – Assisted Living Facilities, Skilled Nursing Facilities, Rest Homes

ACT provides services to LTCFs throughout North Carolina.Our practice of mobilizing clinicians and providing services directly on-site or onsite via Telehealth (face to face through the computer) allows skills to be brought directly to individuals in need.

 

This mobilization has numerous advantages:

  • Clinicians can tailor services directly to the needs of the individual
  • The patient is treated in a familiar setting
  • Patients do not confront the stress of traveling to a doctor’s office and waiting for services
  • Individuals who have trouble arranging transportation will not have to forego needed treatment
  • On-site services allow for better coordination of care between caregivers of the individual in need

Medical Directorship:

Medical Directorship services are tailored to fit the needs of your individual facility.
The basic service package includes:

  • Assume the administrative authority for implementing medical services to include policies and procedures and maintaining the clinical organization of the Nursing Home.
  • Coordinate medical care and maintain effective liaison with attending physicians.
  • Ensure that the medical regimen is incorporated in the resident care plan.
  • Ensure that there are properly written orders upon admission of a resident.
  • Review and sign all accident/incident reports referred by administration.
  • Assist with evaluation of routine fitness of duty for employees.
  • Participate in the following programs:
    • Quality Assurance
    • Risk Management Safety
    • Pharmacy 
    • Infection Control
    • Continuous Quality Improvement
    • Other as necessary and appropriate
  • Review written reports of surveys and inspections and make recommendations to the Administrator.

Primary Care services include:

  • Evaluation/Consultation of Medical Needs
  • Diagnostic Test Determination
  • Medication Management
  • Wellness Clinics
    • Immunizations for Flu/Pneumonia
    • Hypertension/Diabetes Screening and Management
    • Cholesterol Screening/Management
    • Smoking Cessation Counseling
    • Obesity Counseling
  • 24 hour on-call provider

Psychiatry services include:

  • Assessments/Evaluations
  • Physical Examination
  • Medication Management
  • Ordering and Interpreting of Lab Studies
  • Patient/Caregiver Education
  • Coordination of Care with Other Providers
  • Urgent Care Services
  • Participation in Facility Quality Assurance/Quality Improvement Meetings

Psychotherapy services include:

  • Evaluations
  • Psychotherapy (cognitive, behavioral, supportive, counseling, Hypnotherapy)
  • Behavior Management and Care Planning
Between Visits

Our Triage Department can handle many routine needs by phone, and always have access to a Psychiatrist. If there is an urgent care need that requires an on-site visit, arrangements can be made to have an ACT clinician come to your site. ACT is not an emergency service, but we will assist in the handling of emergencies.

Enhanced Service Agencies/Behavioral Health Providers

ACT provides services to Enhanced Service Agencies/Behavioral Health Providers throughout North Carolina. These services are delivered directly on-site or onsite via Telehealth (face-to-face through the computer).

 

Psychiatry services include:

  • Medication Management
  • Diagnostic Assessments
  • Comprehensive Clinical Assessments
  • Person Centered Plan Review
  • Medical Directorship

Psychotherapy services include:

  • Counseling patients through intervention such as cognitive behavior therapy
  • Helping patients improve their coping skills
  • Counseling family members in managing/supporting their loved ones

Telehealth is an alternate way to receive onsite mental health services when a clinician is not physically available in your area. ACT is able to reach the most remote areas of North Carolina by utilizing this technology. Individuals who would have to travel an hour or longer to see a mental health provider are able to go to their local community support provider and have services delivered to them there.
